WYDOT Documentation and Coordination
OpenGate can provide WYDOT-related documentation or coordination when connected to an applicable OpenGate service and supported by a valid case-specific authorization or other verified legal authority. It is not a separate service. Clients should provide the applicable WYDOT notice, letter, form, or request. WYDOT determines driver-license requirements and decisions.

Applying for a WYDOT probationary license?
Have the Alcohol Evaluation and Proof of Compliance for Probationary License from your WYDOT packet available. It documents the evaluation, the recommendations and your agreement to follow them. The provider or providers involved complete the applicable provider sections.
Damon completes documentation for services OpenGate provides. Depending on the case, OpenGate returns the completed form to you or sends it directly to WYDOT with appropriate authorization.
You are responsible for completing your portions of the packet and ensuring WYDOT receives all required materials. Follow the submission instructions and deadlines in your packet.
Submitting the form does not replace completing required education or treatment. WYDOT determines eligibility and makes the licensing decision.
Appointment Options
- The applicable service is available by secure video telehealth (online) and in person at the Casper office, by appointment only. No walk-ins.
- Audio-only telephone may be used for any service or portion of a service when Damon determines it is clinically appropriate and consistent with applicable law, WDH rules and requirements, licensure, professional standards, and the client's circumstances.
- OpenGate provides services by secure video telehealth (online) to adults physically located anywhere in Wyoming. For adults physically located in Colorado, OpenGate provides eligible Wyoming services by secure video telehealth (online) when the requested service falls within Damon's Colorado LCSW scope. OpenGate does not provide Colorado-approved DUI Level I or Level II education or treatment.
- For appointments by secure video telehealth (online), identity and physical location are verified at each session.
The Process
Review the WYDOT request
Share your packet, instructions and deadlines with Damon.
Evaluation and records review
Complete the requested evaluation or provide existing evaluation and service records for Damon’s review.
Prepare the documentation
Damon documents the applicable evaluation, recommendations and service status. Documentation may be required before all services are complete.
Follow through
Meet your education or treatment requirements and WYDOT deadlines. OpenGate provides applicable updates and completion documentation.
When appropriately authorized or otherwise permitted or required by law, OpenGate may provide relevant information to an authorized recipient. Additional federal restrictions may apply to records received from a program subject to 42 CFR Part 2.
A request, referral, deadline, subpoena, attorney communication, probation/parole request, or WYDOT request is not automatically disclosure authority. OpenGate uses a valid case-specific authorization or other verified legal authority before providing information.
