What the comprehensive evaluation includes
  • Comprehensive biopsychosocial and spiritual assessment
  • Administration of the full Addiction Severity Index
  • Application of The ASAM Criteria, Fourth Edition, including applicable dimensional criteria
  • Review of applicable DSM diagnostic criteria
  • Review of relevant records and authorized collateral information
  • Review of blood-alcohol and driving-record information when applicable to an impaired-driving matter
  • Assessment of case-management, referral, and additional service needs
  • Diagnosis and diagnostic summary, when applicable
  • ASAM level-of-care recommendation
  • Written report containing findings, recommendation, clinical rationale, and appropriate next steps
The evaluation process

  1. Contact OpenGate

    Call or email Damon to discuss your ASI evaluation and any court date or deadline.

  2. Intake and records

    Complete intake documents and provide the case information and relevant records OpenGate requests.

  3. Complete the clinical interview

    Participate in the interview, including the full ASI. Complete any necessary follow-up.

  4. Damon reviews the information

    OpenGate reviews the assessment information, relevant records, and authorized collateral information.

  5. Findings and recommendations

    OpenGate determines the diagnosis, when applicable, and makes the appropriate ASAM level-of-care recommendation.

  6. Receive the written report

    OpenGate provides the report to you and other authorized recipients under applicable privacy requirements.

Appointment options, Colorado eligibility & telehealth

Appointments are available by secure video telehealth (online) to adults physically located anywhere in Wyoming. In-person appointments are available at the Casper office by appointment only. No walk-ins.

OpenGate provides eligible Wyoming services by secure video telehealth (online) to adults physically located in Colorado when the requested service falls within Damon’s Colorado LCSW scope. Eligible Wyoming matters include a requested evaluation, service, or documentation connected to a Wyoming court, attorney, probation/parole office, WYDOT, or another Wyoming referral source. OpenGate does not provide Colorado-approved DUI Level I or Level II education or treatment.

For appointments by secure video telehealth (online), identity and physical location are verified at each session.

Audio-only telephone may be used for any service or portion of a service when Damon determines it is clinically appropriate and consistent with applicable law, WDH rules and requirements, licensure, professional standards, and the client's circumstances.

Common questions

What is a substance use evaluation?
A substance use evaluation is a comprehensive clinical assessment of an individual's substance use, functioning, diagnosis, risks, needs, and appropriate level of care. At OpenGate, the full Addiction Severity Index is administered as one component of the evaluation.
What if my referral uses a different name for the evaluation?
Referral sources may use terms such as “substance use assessment” or “substance abuse assessment.” OpenGate’s formal service is the Comprehensive Alcohol and Drug Evaluation. The wording of a referral does not by itself establish the required scope or documentation. Contact OpenGate to discuss the requested service and obtain instructions for securely providing any written referral or requirement.
How long does the evaluation take?
The clinical interview is typically completed in one session. Additional time or a follow-up session may be needed for records, collateral information, or clinical clarification.
What information should I provide?
Have a government-issued photo ID available and provide any available court paperwork, incident records, prior evaluations, treatment records, and other relevant documents requested by OpenGate.
What happens after the evaluation?
Completing an evaluation does not automatically mean treatment will be recommended. If the evaluation recommends Prime For Life® Individual Adult Education, OpenGate can provide that education. If the evaluation recommends Outpatient Counseling (ASAM Level 1.5), OpenGate can provide that counseling when Level 1.5 remains clinically appropriate. Clients are also free to choose another qualified provider, subject to any applicable requirements of the court, probation/parole authority, WYDOT, or other governing authority.
Who receives the report?
The report is provided to you and other authorized recipients in accordance with applicable privacy requirements.
